Cancellation & No Show Appointment Policy
Due to an increase in cancellations,we have issued a new policy that requires all patients to provide a valid credit card upon making your appointment or have one on file. In the event that you do not comply with this policy, your credit card will be automatically charged before you can schedule your next appointment.
CANCELLATION & NO SHOWS
We understand that there are often legitimate reasons for having to cancel an appointment. We ask you to show consideration by calling in advance if you are unable to keep an appointment as we would like to have the option to offer that appointment to another patient who needs to be seen. Failure to provide a 24-hour notice of cancellation will result in a $50 non-refundable cancellation fee for all appointments. Our office is closed on Saturday & Sunday. Should your appointment be on the following day, we would like to ask that you call during office hours for the 24-hour policy to take effect. Failure to receive an appointment reminder notification will not exempt you from this fee. It is the patient responsibility to be aware of their appointment date and time. ASAP REHAB AND PERFORMANCE has the right to hold an appointment for up to 15 minutes pat the scheduled time. In that event that the patient arrives 15 minutes or later past their appointment time, they may be charged a $50 FEE to the credit card on file and may be required to reschedule the appointment for another day.
I acknowledge that ASAP REHAB AND PERFORMANCE reserves the right to charge a fee for missed appointments or procedures. A missed appointment is defined as failure to show for your scheduled appointment or a cancellation/reschedule within less than 24 hours of the appointment time slot.
